Social/Medical Records
To receive a copy of your de-identified social/medical information from your file held at Colorado Christian Services, follow these four steps:
1. Download, complete, print and notarize this PDF form:
Request for De-identified Social and Medical History
The information that will be provided to you will be drawn from Colorado Christian Services’ records and is based on what was given at the time of pregnancy/birth/relinquishment/adoption. Any current or updated information that the agency has will also be provided to you as well.
2. Your request for de-identified social and medical history only fee — with no search — is $50. Please make your check payable to Colorado Christian Services.
3. Provide a copy of your valid, photo I.D.
4. Please mail your request to: Colorado Christian Services, 3959 E. Arapahoe Road, Suite 200, Centennial, CO 80122. Upon receiving your completed and notarized request form, a copy of your photo I.D., and $50 payment for this service, you will be contacted by one of our representatives regarding the next steps.
Reunification
To utilize Colorado Christian Services to initiate a search for members of your adoption triad [birth parents, adoptive parents and adoptee] you must complete these four steps:

2. The fee to conduct a search of a birth parent or adoptee is $75.
If you choose to only obtain your social and medical information, the fee is $50.00. If you then decide to start the reunification process after obtaining your social and medical information, you will need to pay an additional fee of $25, totaling $75. All fees are due prior to services being provided and do not guarantee an outcome of said services. Please make your check payable to Colorado Christian Services.
3. Provide a copy of your valid, photo I.D.
4. Please mail or deliver your request to: Colorado Christian Services, 3959 E. Arapahoe road, Suite 200, Centennial, CO 80122. Upon receiving your completed and notarized forms, a copy of your photo I.D., and associated payment for services, you will be contacted by one of our representatives regarding the next steps.
It’s important for you to know that no one can guarantee the outcome of initiating a search process such as this and that Colorado Christian Services or its agents cannot be held responsible for the outcome of this process or truths that are revealed after this process has begun.
In addition, Colorado Christian Services might not be able to locate the party member that you are requesting and/or if that party member is found, they might not want to reestablish contact with you. The party member sought might also not be living. Colorado Christian Services or its agents cannot be held liable for any results found within the search.
Obtaining Original Birth Certificate
As of January 1, 2016, it is possible for members of the adoption triad [birth parents, adoptive parents and adoptee] to obtain a copy of an original birth certificate through Colorado Vital Statistics. You are encouraged to contact Colorado Vital Statistics directly at: https://cdphe.colorado.gov/adoption-information.
